tommorris basically, just need a template that you wrap the img url in.
{{external image|http://example.org/foo.jpg}} and it then does a hash function on the URL, downloads the image from there, stores it at (hash url).[jpg|gif|png] and then periodically re-pings the site to see if the content has changed
