sknebel the top table (http://microformats.org/wiki/existing-rel-values#formats) references only HTML4 for now and points "proposed HTML5" values to a different table, but HTML5 includes (some of) those too and now defined the <form> case for them. I'd thus add a "effect on <form>" column to that top table too, and references to WHATWG in addition to the HTML4 references?
